The Coinbase One Claim Form is form that #OnchainSummer participants can fill out to receive a 60-day free trial of Coinbase One. Participants must have minted an Onchain Summer release during the event to be deemed eligible.

How did Coinbase use DeForm to create their Coinbase One claim form?

Coinbase created their Coinbase One claim form using various DeForm features. In the above screenshot, you’ll notice that the form was customized using the following features:
- Custom domain (available under the Settings tab of the DeForm editor): https://cb1.onchainsummer.xyz/
- Custom form profile image to reflect Coinbase One branding.
- Custom form background to reflect Coinbase One branding.
- Customized form title.
- NFT access gate: Users must connect their wallet and have minted an Onchain Summer release to be eligible.
In the next image below, we’ll look at what happens once a responder connects a valid wallet that meets the criteria.
Once a responder has satisfied the minimum requirements, they will be able to progress to the claim form. Notice the screenshot above and observe the following DeForm features:
- Customized form description so responders will know what the form’s purpose is.
- Email address question so that responders can claim their 60 days of free Coinbase One immediately after submitting.

After the responder has entered in their email address and pressed submit, they will see the page shown in the screenshot above. This utilizes the following DeForm feature:
- Customized success page which allows the responder to receive a promo code for 60 days of free Coinbase One!
Additionally, once claimed, the responder will receive an email as shown below.
The above screenshot is an example email of the following DeForm feature:
- Email receipt: After the form has been completed, responders will receive a receipt of their form response via email. (This setting can be turned on under the Settings tab of the DeForm editor)
