The CloudBase Application Form allows the interested applicants to apply to be a part of the CloudBase community. Using this form, CloudBase will be able to expand its community and development efforts in the future.

How did CloudBase use DeForm to create their application form?

CloudBase created their CloudBase Application Form using various DeForm features. In the above screenshot, you’ll notice that the form was customized with the following:
  • Custom form profile image to reflect CloudBase branding.
  • Custom form background to reflect CloudBase branding.
  • Customized title and description so responders will know what the form’s purpose is.
  • Connect Wallet function to verify responder’s onchain activity.
  • Short text questions asking for responder’s name or pseudonym as well as the name of their projects.
Although it isn’t pictured in the above screenshot, CloudBase also customized their form with a cover page (as seen at the top of this blog post!) to welcome potential responders to their form.
Let’s move on and analyze the next section of the form!

In the above screenshot, you’ll notice the following features:
  • Long text questions to give responders a chance to submit project information and a list of backers, partners, or advisors.
  • Single select question inquiring each responder about their team members and whether they are anon, fully public, or a mix of both.
  • Short text questions asking responders about the current status of their projects and whether or not their smart contracts have been audited.

In the final section of their form, the following DeForm features were used:
  • Link questions to gather each responder’s links to things like their project’s litepaper or whitepaper, project website link, Medium link, and GitHub link.
  • Twitter Connect so that responders can optionally verify their Twitter account.
  • Telegram Connect so that responders can optionally verify their Telegram handle.
  • Discord Connect so that responders can optionally verify their Discord handle.
