The TBW Conference Survey is a form which interested individuals could provide information that would be used as feedback for Taipei Blockchain Week. This feedback would be useful so that TBW staff can further understand its attendees’ demographics, interests, and aspirations.

What is DeForm?

In the above screenshot, you will notice that this form was created using the following DeForm features:

Two multiselect questions (One with a maximum of 6 selections and one with a maximum of 3 selections) were used to analyze each applicant’s web3 passions and interests.

One single select question that asks each applicant how long they’ve been in web3 or the blockchain industry.

In the screenshot above, you’ll notice the rest of the form uses a few different question types, including:

A single select question to ask applicants what region they’re from.

A multiselect question that asks each applicant what they’re hoping to get out of the TBW event.

Short answer questions to collect each’s applicants Telegram and/or Twitter handle(s) (in case they win a raffle).

A Wallet Connect question to collect each applicant’s wallet address (in case they win a raffle)
